Sarah is a Doctor of Physical Therapy with advanced training in orthopedic, sports, and post-surgical rehabilitation. Her practice, Team Rally Sports Physical Therapy, focuses on helping athletes and active individuals return to sport with confidence through individualized rehab, objective testing, and performance-centered rehab. She is specialized in ACL and knee injuries but treats all types of sports injuries.

Team Rally Services
Team Rally offers two distinct categories of care: Physical Therapy and Return-to-sport and Injury Prevention (“Prehab”) Coaching
Each is structured to meet the specific needs of athletes and active individuals at different stages of recovery and performance.
Physical Therapy Services
One-on-one, evidence-based care provided by a licensed physical therapist. These services are designed for individuals experiencing pain, injury, or physical limitations requiring skilled rehabilitation.
ACL Rehabilitation
Post-operative and non-operative care for ACL injuries
Strength, mobility, and neuromuscular re-education to return to jumping, cutting and pivoting at a high level
Objective progress tracking and return-to-sport testing, including force plate analysis
Options for both intensive, in-person, hybrid, and fully remote care
Sports Injury Management
Evaluation and treatment for acute and chronic musculoskeletal injuries
Focus on load management, tissue healing, and performance-based recovery
Return-to-activity progression based on sport demands
Includes objective testing to inform progression and monitor recovery
Use of compression therapy, instrument-assisted tissue mobilization, and manual therapy for acute management of symptoms
Post-Operative Rehab
Structured recovery programs following any orthopedic surgery
Emphasis on restoring range, strength, and movement quality
Use of objective measures (including force plates, strength testing) to guide rehab
Collaboration with surgeons and care teams as needed
Note: Physical therapy services are available only on a cash-pay basis. We do not bill commercial insurances. At this time we are unable to work with patients using Medicare or Medicaid, but this may change in the future.
Return-to-sport and Injury Prevention (“Prehab”) Coaching
These services are for teams, athletes and active individuals who are not seeking treatment for an injury, but want structured support to reduce injury risk or transition from rehab to full activity after discharge from formal therapy.
Return-to-Sport Programming
For athletes completing or transitioning from formal rehab
Programming built around sport-specific movement demands and your personal injury history
Periodic force plate, hop, and strength testing to track improvements
Focus on readiness, restoration of athleticism and movement quality-not injury care
Injury Screening and “Prehab” Testing
Screens movement quality, strength testing of muscle groups, and unilateral jump testing to look for side-to-side asymmetries
Ideal for baselining, identifying “prehab” targets, and return-to-sport decision making
Detailed reports provided for athletes, coaches, and/or parents
Team Injury Screening & and injury prevention education
On-site or in-gym team screenings using strength and movement metrics
Individual injury risk profiling and sport-specific insights on high-risk injury areas
Instruction in ACL prevention warm-ups to prevent injuries
Disclaimer: Injury Prevention and wellness services are non-clinical and not a substitute for physical therapy. These services do not include evaluation or treatment of medical conditions, injuries, or pain. Clients must be medically cleared for physical activity. These services are not eligible for insurance reimbursement and are not covered by Medicare or Medicaid, however these clients may pay privately for these services.
Access for Medicaid (Apple Health) Patients
While Team Rally Sports Physical Therapy is not enrolled with Medicaid and cannot bill for covered medical physical therapy services at this time, we remain committed to supporting all athletes in our community.
Medicaid beneficiaries are welcome to participate in non-covered performance and wellness programs, including return-to-sport coaching, recovery sessions, injury prevention, and performance testing once you are no longer in formal physical therapy. These services are paid privately, and we offer a pay-what-you-can option to help make participation more accessible. Please contact us to discuss what is affordable for you-we are happy to work with your budget.
